Recovery after surgery can limit a person's ability to complete their daily activities. Hospital Discharge Services will provide a seamless transition from hospital to home, ensuring the individual has everything they need, allowing them to focus on recovery. Services may include: organizing community resources, case management, and the provision of assistive devices.
Attendant Care Assessments will determine the level and type of care an individual requires to perform routine personal care tasks in the home, safely and effectively.

The Assistive Devices Program (ADP) in Ontario provides funding for individuals with long-term physical disabilities with customized equipment and supplies (if eligible). Our team can assess an individual’s function and needs, and prescribe appropriate devices such as wheelchairs and mobility aids.

Our team of OTs can assist with gradual return to work programs, work hardening programs, job coaching, symptom management and ergonomic assessments and education. Rehabilitation for driver anxiety involves a combination of therapeutic approaches to help individuals regain confidence and comfort while driving. This program includes a combination of the different therapies that are tailored to the individual’s specific needs. It also involves on-road practical driving sessions with a trained driver rehab instructor to build confidence and skills.
